General Summary of Position:
As a member of the Health Services team, the Nurse Care Manager and Outreach Coordinator is responsible for providing broad case management services associated with specific student health care needs including but not limited to support of student referrals to specialty services outside of UML and to disease specific logistical support during times of public health initiatives. Triage and direct nursing care to students are also primary functions of the role.
Position responsibilities include:
Provides support of outside health care provider referrals:
- Telehealth or in person visit with student to review orders from UML NPs for referrals including but not limited to determination of access to the provider due to insurance coverage, and follow up for student return to care of UML HS provider
Triage of student calls associated with reportable communicable disease:
- Assesses concern by accurately gathering subjective/objective data and medical histories
- Serves as liaison to local public health departments and/or designees as directed by Director or designee
- Provides appropriate nursing advice or treatment and plan of care per scope of practice including clarification of self-care directions
- Documents each encounter accurately in medical record
- Consults with nurse practitioners as needed
- Communicates to Director or designee of potential public health issues on campus and reports issue to appropriate authorities
Provides direct nursing care to students:
- Provides individual health counseling
- Administers vaccines as necessary, particularly during peak demand periods
- Collects lab specimens and performs CLIA waived laboratory tests as directed and available
- Accurately documents each encounter in medical record
- Supports immunization compliance activities to facilitate compliance and subsequent access to class registration
Support of LTBI assessment process:
- Maintains up-to-date LTBI Risk Assessment and Symptom Review forms
- Administers and interprets Tuberculosis skin tests as necessary with required documentation, referral, reporting and follow up plan of care
- Obtains specimens for IGRA TB testing as appropriate with required documentation, referral, reporting and follow up plan of care
Administrative:
- Generates reports as required and/or requested to facilitate accurate documentation and clinical work flow for reportable communicable disease
- Coordinates scheduling and administration of annual influenza clinics and other public health initiatives
Minimum Qualifications (Required):
Education:
- Graduation from an accredited school of professional nursing, with an associate degree in nursing, bachelors degree strongly preferred
Certifications/Licensures:
- Current R.N. licensure with the Massachusetts Board of Registration in Nursing
- Current BLS certification
Experience:
- Minimum of 3-5 years of professional nursing experience, in an acute care, urgent care, or primary care setting
Preferred Qualifications:
- Bachelor's degree from an accredited school of professional nursing
